We are a dynamic, high-energy team dedicated to making a difference in the lives of adults across our community. As both a Home Health and a Program Approved Service Agency (PASA), we provide a wide range of services tailored to meet the diverse needs of those we serve.

Our mission is to uplift, support, and empower individuals to live fulfilling, independent lives. Whether we’re helping with daily personal care or connecting clients to the community through engaging day programs, we are passionate about providing top-quality care with heart.

Our Services:

PASA Services:

IHHS (In-Home Support Services): Personalized support that helps adults live comfortably and independently in their own homes.
Pre-Vocational Services: Preparing individuals for meaningful employment opportunities through skill-building and training.
Residential Host Home: Creating safe, nurturing living environments with dedicated host families.
Supported Community Connections Day Program: Engaging, community-based activities that foster social connections and personal growth.

Home Health Services:

Personal Care: Compassionate assistance with daily activities like bathing, grooming, and mobility, skin care, medication reminders and toileting/incontinent care.
Homemaker Services: Helping maintain a clean, safe, and organized home environment, meal planning & preparation,, laundry, prescription pickup and errands.
Alzheimer's and Dementia care, Hourly care, 24hour & live-in care and respite care.
