Our director is well-versed in both residential agencies and CCBs, bringing a unique perspective and experience to our organization.
Our Residential Managers add even more knowledge and experience to our team, ensuring we provide the best possible service to our community and the individuals we serve. Together, we form a formidable team that is dedicated to delivering the best care possible.

We are a Program-Approved Home Help Service Agency that is dedicated to improving the lives and well-being of individuals receiving home and community-based waiver services. Our commitment to these individuals is to provide services that enrich their lives, teach them important independent skills, and create a safe, supportive environment for them to live in. Our program focuses on Residential Care, Pre-Vocational Services, Supported Community Connections, and Non-Medical Transportation. We strive to make sure that our clients have the ability to live independently and as happily as possible.

The staff excell at assistancing adults and seniors to continue to live safely and independently in the comfort of their home. Extended Care LLC offers individual residential services and support for up to two individuals in a single residential host home setting.
Host Home Setting
The Host Home model provides an alternative to traditional apartment or group home living.
Supported Community Connections
Our SCC community integration program provides individuals with a range of activities to help them become more integrated into their local community.
Non-Medical Transportation
Extended Care LLC provides transportation services to assist our individuals in connecting with their local community.
Pre-Vocational Services
Pre-Vocational services is designed to assist individuals with developmental disabilities in acquiring and maintaining work habits and job skills.
Non-Medical Home health
Extended Care provides personal care services that assist adults and seniors in their homes. We assist with activities of daily living, such as bathing, dressing, toileting and in some cases light housekeeping.
