Welcome to Extended Care Services, where compassion, independence, and community come together.

We are a Colorado-based Class B Non-Medical Home Care Agency and Program Approved Service Agency (PASA) dedicated to helping older adults, veterans, individuals with disabilities, and families live safe, meaningful, and independent lives. Our team provides personalized support to older adults, individuals with disabilities, and families through a wide range of home and community-based services.

Our services include non-medical home care, Community First Choice (CFC), homemaker services, personal care, respite care, Host Home services, Supported Community Connections (SCC), Mentorship, Pre-Vocational services, Family Caregiver supports, and Non-Medical Transportation. Whether supporting an individual in their home, helping them build community connections, or promoting greater independence, our focus remains centered on the unique needs and goals of each person we serve.

At Extended Care Services, we believe every individual deserves to be treated with dignity, respect, and compassion. We work closely with individuals, families, guardians, healthcare professionals, and referral partners to develop personalized support plans that encourage independence, community involvement, safety, and overall well-being.
