Residential Services
Extended Care LLC offers individual residential services and support. PCA and Host Home options are designed to support 1-3 individuals in services living together. PCA settings are flexible environments that can be individualized to meet each person’s support needs. Settings may include the person’s own home/apartment or an agency owned/leased property. PCA settings may also include Family Caregiver (FCG) environments in which persons choose to receive residential services in their family home. Host Homes are single-person caregivers, couples or families who open their home to a person with IDD. People live in the home and receive support to live independently in the community and home of their choice.
Our experienced team provides personalized placement and support for individuals to develop living, social, and independent growth skills. Our dedicated staff is committed to creating a safe and secure environment for individuals to thrive. With our support and services, we are confident that individuals will have the opportunity to reach their highest potential.
Host Home Setting
The Host Home model provides an alternative to traditional apartment or group home living. It allows an individual to live in a private residence with a family or caregiver (non-related parent, guardian, or spouse). Host Home Providers are independent contractors who manage their own home businesses. The Host Home approach is highly flexible, providing a person-centered care model that is tailored to the individual’s needs. With this model, the individual has the opportunity to live in a safe, comfortable, and supportive home environment.
Our agency is here to support individuals in achieving their individualized goals. We provide comprehensive services such as BISSP and ISSP, and long-term planning. We are committed to helping individuals reach their goals by providing person-centered approaches and training our providers on these goals. Additionally, we monitor goal tracking to ensure that individuals are making progress in the necessary areas, and report back to the IDT members.
Supported Community Connections
Our SCC community integration program provides individuals with a range of activities to help them become more integrated into their local community. From bowling, miniature golf, and outdoor sports, to flying kites and attending art shows, we have something for everyone. Through our activities, we strive to provide opportunities for our clients to make new friends, gain new skills, and explore their interests.
Non-Medical Transportation
Extended Care LLC offers transportation to supported community connections and employment sites, as well as interviews, to help our individuals reach their goals. We understand that transportation can be a barrier to success, but we are here to make sure our individuals have the support they need to succeed. Let us help you get to where you need to go!
Pre-Vocational Services
Extended Care LLC provides one-on-one and group training for safety in both the workplace and the community. We help individuals learn how to properly communicate, complete tasks, make decisions, problem-solve, ask questions, and self-advocate. We also provide resources and assistance to help individuals use public transportation and read bus and train schedules. For those unable to use public transportation, we will teach and accompany them to and from work until they are comfortable doing it on their own.
Furthermore, our Pre-Vocational services provide a monthly calendar to our individuals to prepare them for their daily schedule and to help build skills for employment. We have many community activities and volunteering opportunities to help further develop these skills. All Pre-Vocational services will be conducted in a safe and supportive environment to ensure the best possible outcomes.
Family Care Giver
PCA settings may also include Family Caregiver (FCG) environments in which persons choose to receive residential services in their family home. Host Homes are single-person caregivers, couples or families who open their home to a person with IDD. Persons live in the home and receive support to live independently in the community and home of their choice. Family caregivers can be reimbursed for services provided to their family member in two ways. A family member can be hired as a care provider by a Program Approved Service Agency (PASA) and must meet the same requirements as other employees. Family members may also explore becoming an independent contractor if they meet IRS independent contractor criteria.

We offer a wide range of home care services that cater to the health and wellness needs of your loved one. From 24-hour care to hourly care, we can help you work through it all. Additionally, we offer flexible scheduling. No matter what hours you are available, we can find caregivers that perfectly adhere to your schedule.
Non-Medical Services:
The essential home care services that we offer include:
• Handling light housekeeping around the home
• Making/planning delicious and nutritious meals
• Assisting with hygiene or grooming related tasks
• Mobility assistance
• Providing transportation and companionship to appointments and errands
• Reminding seniors to take their medication.

Respite Care Services:
Our caregivers will help relieve family caregivers for a short period of time.
Providing all of the normal supports as their primary caregiver.
