Extended Care Services partners with families, guardians, case managers, and community professionals to connect individuals with quality Host Home, Supported Community Connections (SCC), Mentorship, and PASA services throughout Colorado.

We understand that every individual has unique needs, goals, and preferences. Our team works closely with referral sources to ensure a thoughtful and person-centered matching process.

Host Home Services

Family-style residential supports in a safe and supportive home environment.

Community First Choice (CFC)

Personal care and homemaking services that help eligible Medicaid members remain safe, independent, and comfortable in their own homes while receiving support with daily living activities.

Home Care Services (Class B Non-Medical Home Care)

Non-medical support designed to help individuals maintain their independence at home through assistance with personal care, homemaking, companionship, meal preparation, and daily living activities.

Supported Community Connections (SCC)

Community-based activities designed to promote independence, social engagement, and 

personal growth.

Mentorship Services

Individualized support focused on skill development, goal achievement, and community participation.

Pre-Vocational Services
Skill-building opportunities that prepare individuals for future employment through hands-on learning, workplace readiness, and career exploration activities.

Family Caregiver Services
Support that allows eligible family members to provide care while promoting stability, continuity, and person-centered support.

Respite Services
Temporary relief for caregivers while ensuring individuals continue to receive quality support in a safe and familiar environment.

Non-Medical Transportation (NMT)
Reliable transportation that helps individuals access community activities, employment opportunities, appointments, and other approved destinations.
